Essential Requirements

  1. Valid passport last 6 months,
  2. One visa application form online filled & take back to back print,
  3. Two colour photographs 35m by 45mm 80% face should be seen white back ground,
  4. Covering letter on company letter head addressing to the Poland consulate Mumbai,
  5. Hotel confirmation from Poland 1 copy fax directly to the consulate & 1 copy to us,
  6. Overseas Medical Policy,
  7. Air ticket,
  8. Passenger's personal IT last 3 years OR Salary slip last 3 months,
  9. Passenger's personal bank statement last 6 months,
  10. If Employee than required Leave letter from company,
  11. International credit card if Passenger have.

  7. Air ticket, Passenger's personal IT last 3 years OR Salary slip last 3 months,
  8. Passenger's personal bank statement last 6 months,
  9. Appointment letter copy,
  10. International credit card if Passenger have.

  9. Overseas Medical Policy.

Notes

Please Note:- we have to take the appointment for submission at consulate for all Visas and Agents can submit if Applicant already Provided Bio Metric within Last 5 years In that also only Business visa applications & Tourist visa applications and For all other categories pax have to come personally for submission at consulate with prior appointment.
